Visually graded condition of disc: A: Near Mint minus / B: Near Mint (unplayed).
Side One has some very faint hairline marks more than likely from the inner sleeve from inspecting this disc many times over the last 40 years. In no way will they affect play. Cut 3 on Side One had a small spot mark - I checked it with my V15 stylus and it is not audible. Side two is spotless, so I expect both sides to play like new. 
 

Condition of sleeve: Shows some wear because it's white. Light rings on both sides. Small stain on backside at lower right is actually the glue used in manufacturing, not from any abuse. It won't come off. You may be able to find something that will work, but I'm not trying. Tiny stain on backside at left just above center. Tiny promo slit at top left on spine. Top right corner has a little wear - starting to split but is only 1/8" long, hardly worth mentioning. Promo sticker on front.

MY GRADING SCALE
(very similar to the Goldmine standard)

NM (Near Mint): Record will show no serious scuffs or scratches in the playing area. May have minor scuffs on edges from decades of storage, or a small scuff, scratch or paper scuff on playing area.
It's near mint, not mint (which would be 'sealed' to me). There should be no audible distractions.
M-/NM- (Near Mint minus): A scuff or two or light scratch or two on either or both sides that may be heard.
VG+: Scuffs & some minor scratches in the playing area. Some will most likely be heard.
VG: Many scuffs or scratches that will be heard but record should still play fine.
