EDITION
For purposes of this discussion, first editions are those with the SIX EYE-BOX labels (see the images). There are several different pressings with these labels. This description refers only to these original, U.S. pressings. All editions from 1963 and on have a reformatted label, and are not relevant to this description.
This disc has the Six Eye Box labels. The following discussion attempts to identify which pressing it is, from the information on Discogs.
Besides mono vs. stereo, there are three major factors differentiating between the first pressings (which include the promos) and later pressings: Two items on the back sleeve and the other on the labels, All of these were released in 1962, I believe close together.
On the sleeve, first pressings have the words "Produced by John Hammond" prominently displayed directly underneath the album title on the back cover. It was removed in some of the later pressings. Additionally, some of the later pressings have the numeral "8" in small type on the lower right corner on the back cover. 
This disc doesn't have the J.H. producer credit, and does have the numeral 8.
Of the original Six-Eye labels format, a slight change was made after the first pressing (which includes the promos). In the first pressings, "Bob Dylan" is printed in two lines above the spindle hole. In the modified second pressings, "Bob Dylan" is written in one line, and directly underneath is the first track of the side, both above the spindle hole. (The remaining tracks are listed underneath the hole.)
This LP has the second, modified labels. [Above the spindle hole: "Bob Dylan" in one line, and directl underneath, the first track.]
In Discogs, the sleeve matches the edition labelled as "No Hammond Credit," as does the label... however... and here's the mystery. The matrix runout does not match. In fact, the matrix runout on this disc does not exactly match any of those on Discogs. It very closely matches the edition labelled as "Repress, Pitman Pressing," which doesn't have the Six Eye Box label (it shows a later edition, Two Eye Box label).

Note: (1) The vertical lines are etched separately on the opposite side of the runout.           (2) The "P" on Side 1 is disfigured. It's possible it may not be a "P". See the closeup images.

fyi, the matrix runout on the "No Hammond Credit" edition from Discogs is: (Side 1)  XSM-55621-1D   H  | (Side 2)  XSM-55622-1D   H  H
which clearly doesn't match.
My take: The "P" on this disc denotes a Pitman pressing?  Does having a slightly different runout make the disc especially rare?
Now you know as much as I do.
DISC HISTORY
I bought it in a used record shop around 40 years ago in California. I can say, for the record (ha ha), that I have only played it twice. The second time was in preparation for this listing, after cleaning it.
There is an ownership stamp on the back cover. Someone in New York. I would guess it's a solid bet they were the original owner.
CONDITION
The LP is in great shape. The original shrinkwrap is mostly intact; a piece is torn off in the front. There is slight ringwear at the bottom of the front. There is about a i-inch slit in the sleeve at the bottom edge corner. The spine is perfect, as is the top edge. Considering the age, I would rate it VG+/EX.
The inner sleeve is in great condition. Not tears or splits. Plastic inside paper.
The disc plays beautifully. Slight surface pops here and there. Overall: EX.
PACKING
I will pack it with the disc inside a Big Fudge Premium Master, acid-free, archive quality inner sleeve, outside the jacket sleeve, enclosed by cardboard padding inside a quality LP shipping box.
