First issue variant two with swirl labels and a polyvlined plain sleeve.
Contains attached four-page photo booklet inside matt gatefold sleeve.
No "Made in England" on label.
On the label the title appears as "Black Sabbath Vol. 4".
A2 appears as "Tomorrow Dream" on cover, but as "Tomorrow's Dream" on label.

Runouts are stamped except for the PORKY / PECKO etchings.
PORKY / PECKO is etched upside down in both sides runouts.
this variant is identified by the following:
• Vertigo spiral/swirl labels.
• Matt gatefold cover with glued-in 4 page insert.
• No track timings inside gatefold.
• No 'Made In England' on label.
• 'E' of 'SIDE B' aligned with the 3 (of the ⅓) above it.
• Ⓟ 1972 slightly higher up, compared to other releases with Ⓟ directly between the two 3s below it;
likewise the catalogue number is slightly higher up, side A tracks numbering has 1. and 2. aligned, with 3., 4., 5. slightly to the left;
some have side B tracks numbering 2. and 3. slightly out of alignment with the remaining.
