DEATH Scream Bloody Gore LP Original 1987 VG+ No Barcode FLAG12 Chuck Schuldiner No barcode on the back cover, plus Under One Flag address is different than the one on the later repress.
I bought this second hand early 90s. Vinyl/Sleeve/Inner Ex/VG+/VG+. I have only played 3/4 times and has been safely stored in my personal collection for 30 years. The vinyl is in excellent condition with only minor surface wear. The inner sleeve has some marks across both sides which were there when I purchased (photos included). 
All vinyl is shipped in heavy duty cardboard record mailers with 2 cardboard stiffeners. Inner sleeve/vinyl will be sent outside the cover within a plastic sleeve.
