Donald Byrd Free Form Blue Note 84118 stereo "NY USA" label

"NY USA" label and "Van Gelder" in the run outs both sides. No "P" aka "The Ear" on any pressing of this title. "43 West 61st., New York 23" address on back cover.
Record looks and plays VG++/VG+. Side 1 looks clean. Side 2 has some light scuffs and a small scratch near beginning of Track 2 which causes a few seconds of light crackle. Most of side plays VG++. No heavy or repetitive scratches or ticks. Writing on label and stamps both sides.
The cover is G+.
The front cover has large writing and heavy wear. The back cover has large writing and moderate to heavy wear. Top seams taped and probably split all the way across. Corner cut at the pocket. Bottom seam split app 3 inches in the middle and untaped. Also split app 2 inches and taped at the pocket. Corners heavily worn and dented.
