This album came from the vaults at WLUP (The Loop 97.9) radio  station in Chicago  which has since closed a few years ago.
On the Grateful Dead record label and the record number is: GD-LA494-G stereo COLUMBIA TERRA HAUTE PRESSING: Denoted by the stamped "CTH" in the dead wax runoff

 The matrix numbers in the trail-off are:
