A fascinatingly rare piece of Henryk Szeryng's recorded output.
But first some information on Thomas L. Fawick...
14 April 1889- 8 January 1978 in Sioux Falls, South Dakota (relocating to Cleveland, Ohio in 1942). Patented over 250 inventions and allegedly built the first 4-door touring car in the United States. 
Other than being an inventor, industrialist and art collector...Fawick was a violinist, luthier and composer.
The selection listed today is listed as having 3 editions published between 1963 and 1968 (according to wordcat...)

The disc clocks in around 3 minutes and 40 seconds, concluding with Henryk Szeryng speaking briefly 
This disc is in unfortunately rough condition so I included a sample of the first 30 seconds or so to give you a feel for the recording. (Copy & paste link please)
I purchased this disc with the adapter placed in the back, as seen in the third photo.
