Label is "767 Lexington Ave NYC" and deep groove on both sides.  "RVG" "9M" and "The Ear" aka "P"  in the runout on both sides. Record is flat edge.  Cover is frame cover without writing on the spine and has "767 Lexington Ave., New York 21" address. 

Record looks VG++, close to M- and plays beautifully at a strong VG++.  Record looks bright and glossy and little played.  A few light scuffs.  No scratches.    

Cover is VG++

 Front cover is bright and still glossy, shows some light wear and looks nice. Spot of heavy wear and beginning of a seam split app 1/4 inch at bottom at the pocket.  Other corners tight and nice.  Back cover bright and nice showing little or no wear. Spine is bright and intact. A really nice and beautiful cover.
