RCA Living Stereo LSC 2222 with shaded dog label disc pressed in the US.  3s 1s stampers.  Debussy, Iberia, Ravel, Alborado del Gracioso, Valses Nobles et Sentimentales.  Fritz Reiner conducts the Chicago Symphony Orchestra.

Disc NM, Jacket VG++  an extremely nice copy

Fantastic sound, one of the best Reiner/CSO Living Stereos, and a very rare disc.
