This is a Metallica "The $5.98 E.P. Garage Days Re-Revisited" picture disc from Vertigo.
Songs include:
Helpless The Small Hours The Wait Crash Course in Brain Surgery Last Caress/Green Hell Eye of the Beholder (bonus track)
The vinyl looks ok, but I can't confirm it's playability. 
The sleeve is in very bad condition (rips, bends, tears, pieces/corners missing). Please see photos. 
Would look cool in a vinyl frame and hung on the wall.
