Both inserts grade VG+ to EX (very light wear with some tiny pin holes through the top corner on both inserts and a small sticker on one of the bottom corners)
The survey card grades NM.
The gatefold sleeve grades EX to EX+ (very light wear with a very small surface tear near the bottom left corner, a very light shadow near the top right corner (and also one on the back near the bottom left corner) where a sticker has been removed).
The obi grades EX+ to NM.
