Just about everything that you would like to know about this obscurity can be found on line. Type "Punk Business Manager: Public Disturbance" on to Web Search. On the 1/6/2008 posting there is even an actual copy of a page from a June 1989 MAXIMUM ROCK N' ROLL MAGAZINE containing a laudatory review of the S+M EP. This EP has it all, the cool cover, the punks (da band) on the back and the slammin' music in the grooves.
The vinyl is Mint and the picture sleeve is M- based upon a few very slight, light wrinkles at the top of the sleeve.
