Peter MAAG conducting the Paris Conservatoire Orchestra:

(UK Decca SXL 2044, 1958)
grade (disc/cover): strong EX (near-mint, really) / NEX
notes on condition: Under bright, direct light the disc shows no discernible flaws or signs of being played/ handled. Sleeve: creases in lamination near opening, ring wear in the form of loss of gloss to laminate; tiny price sticker in upper left corner of back cover; small numbers handwritten on back cvr ("139" in blue ball-point; "34" in pencil); purplish date stamp on back: "DEC 18 1958"

Records are conservatively graded taking into account both how the record looks and sounds. I try to describe the condition of each item as completely as I can; please also refer to the pictures. A NM record or sleeve shows little or no signs of ever having been handled. A disc or sleeve with only the slightest signs of wear and handling, or one that is in NM condition with one or two minor flaws, will be graded EX. My next grade is NEX for items that don't qualify for a strict "EX" but don't show wear consistent with most people's definition of VG+. A NEX record will still have a nice gloss and can be expected to play back extremely well. A VG+ record will show more signs of wear & handling but this can be light scuffing with little effect on playback. A VG record has more wear to both the grooves and the label; it will still deliver an enjoyable listening experience.
