US issue on Blue Note 1581
47 west 63rd on both sides (no address on back cover)
deep groove, no R, RVG and 'ear' etched in dead wax 

Record :  Record shows few light surface marks on both sides (no deep marks) without any effect on sound quality. Visually : EX(-) and plays EX (at least) 

Cover : Light wear on bottom right corner, one surface mark (as if somebody wrote on a paper put on the sleeve) on front visible only by reflection, light surface wear on back, small number on back. VG++/EX(-)
