The Original Sound label always made most of their money by selling the "Oldies But Goodies" [at least 15 volumes] collections of old rock and soul hits. They were started in the late 1950's by California disc jockey Art Laboe who popularized the "oldies but goodies" format on radio KPOP during the "Art Laboe Show".
A small intense label, they had some rarities issued - Kim Fowley, Dyke and the Blazers, etc., great garage-type stuff.
So the company sleeve used on this issue, and and at least four other singles later on by TMM, has head shot pictures of five band members, and graphics from the first LP cover on one side, but the reverse side is simply label advertising - for the first eight OBG albums, plus one by musician Stan Hoffman and vocalist Norma ["Norman" on the sleeve] French.
Many other Original Sound company sleeves exist, with a lot of variations in the albums that are being advertised.  Their promotional department probably made up a new one every few months, but none of the others have a group on it like this one does. Very few of these have surfaced in the last twenty years, maybe 50 all together. It is 'way scarce!
